All Star Absence Form: If you will be missing your all star team practice, please complete the form below at least 2 weeks prior to your absence. Please remember each all star is given 4 unexcused absences throughout the season. If they receive more than 4 unexcused absences, their position on the team will be reviewed.  All Star Absence Form
